Temporary Food Service Booklet

Our mission in the food safety program of the Jefferson County Health Department is to ensure food is handled safely for all populations. This is accomplished by inspecting food facilities, whether temporary or permanent, and educating food handlers.

To learn about proper food safety, just watch the short video below and follow the directions to receive your TB skin test. To receive your food handlers card complete the questionnaire below and click submit. Your food handlers card will be issued when you return to the health department to have your TB skin test read. *You must complete the questionnaire to receive credit for completion of the food handler’s slide show. Please make sure you put your employer’s name and your name at the beginning of the questionaire. If not, the tests that we receive without a name will be discarded.

Once you complete the food questionnaire, you will complete a yearly TB Risk Assessment Form when picking up card. The Food Handlers cards are valid for one year. You must renew food cards every year. There is a $10 fee for your food handlers card and a $5 replacement fee.

Please allow 24 to 48 hours before picking up your food card.

Below are the listed times that you will only be able to pick up your food card and complete your tb risk assessment: Times will vary on holidays

Mondays 9 am — 11 am
Tuesdays 1 pm — 3 pm
Fridays 9 am — 11 am & 1 pm — 3 pm

Jefferson County Health Department will no longer be requiring a Tuberculosis Skin Test (PPD) in order to obtain a Food Handlers Card. Employees must still complete the Food Handlers Questionaire.

The Jefferson County Health Department inspects schools, hotels, nursing home kitchens, restaurants, and temporary establishments, as well as other facilities to ensure that food handling practices are satisfactory.
